Plan fee schedule -
Is the amount a participating FDHaccess Dentist/Specialist will charge a patient. These charges have been negotiated with our dentists and are 25% to 65% less than the dentist's usual and customary fee.
Patient Responsibility -
FDHaccess networks are easy to use. There are no claim forms, you simply pay the discounted network fee to the Dentist at the time of treatment.
